WHO WE ARE
We are an independent nonprofit that works across New Jersey to improve health and well-being.
Our job is to help the people and organizations that do provide services work better together.
WHAT WE DO
We focus on the things that shape health every day, like housing, education, transportation, jobs, and access to care.
We are an independent nonprofit that works across New Jersey to improve health and well-being. We are not part of the government, and we do not provide medical care. Our job is to help the people and organizations that do provide services work better together.
We bring together people, community groups, health providers, schools, local leaders, and government to share ideas, solve problems, and work toward common goals. We focus on the factors that shape health every day, such as housing, education, transportation, employment, and access to care.
Where you live, your income, and how systems treat you should not decide how healthy you can be. We work to make sure every New Jerseyan has a fair chance to be healthy and to get the support they need to thrive.
Our partnership with the New Jersey Department of Health lets us work across systems statewide, not just individual programs, and strengthens our credibility, public trust, and long-term impact.

Health gaps shrink in Health Opportunity Zone communities.Fewer unfair differences in health outcomes where we’re working.
More people have a voice in decisions that affect their health.Communities that have been left out are better informed, more engaged, and heard.
Care and research reflect the communities they serve.Providers use what works, and more people can take part in clinical research, so treatments work for everyone.
